Post-Congressional Meeting Report Form

Thank you for taking the time to report on your meetings with Senators and Representatives.  These reports will help us build a comprehensive picture of the Congressional lobbying efforts that took place during the 2007 Department of Peace Conference.

As we did with the Pre-Conference Survey, we are asking that you complete a separate report form for EACH meeting that took place.

Notes:

  • Information with asterisks (*) is required. You cannot complete submission if they aren't filled in.
  • We do not need every person attending the meeting to complete the form, but we want to hear from everyone who wishes to report.
  • We suggest that you coordinate with your state leaders to make sure that each meeting is recorded at least one time.
