Post-Congressional Meeting Report Form

Thank you for taking the time to report on your meetings with Senators and Representatives. These reports will help us build a comprehensive picture of the Congressional lobbying efforts that took place during the 2007 Department of Peace Conference.

As we did with the Pre-Conference Survey, we are asking that you complete a separate report form for EACH meeting that took place.

Notes:

Information with asterisks (*) is required. You cannot complete submission if they aren't filled in.
We do not need every person attending the meeting to complete the form, but we want to hear from everyone who wishes to report.
We suggest that you coordinate with your state leaders to make sure that each meeting is recorded at least one time.
